Medical Concierge has simplified the admission process to our behavioral health center so that the patient and their family may have a stress-free, streamlined transition into treatment.
Pre-admission Assessment: Each prospective patient undergoes a pre-admission assessment to build the patient profile, including medical history and background, symptoms, mental illnesses present, medications used, etc. This is conducted by a treatment consultant either over the phone or in person at the mental health treatment center. Once the pre-assessment is complete, the treatment consultant and the patient can set a mutually suitable admit date.
We understand that the patient and their family may have various expectations and questions about the mental health treatment program. The pre-admission process is an appropriate time to address these with our treatment consultant, who can answer all of these and put the patient and their family at ease.
Admission: The treatment consultant associated with our mental health treatment center will schedule an admit date for the patient to arrive and begin treatment. The patient is welcome to bring a close friend, loved one or family member to help ease the fears and support them in this transition. At this time, they will take a tour of the behavioral health center and gain more insight and understanding as to what to expect during the treatment process. In addition, the admission paperwork will be completed, and protocols and rules will be explained.
Treatment: Within the first 24 hours of admission, the patient will be introduced to their therapist who will develop an individualized treatment plan most suitable to the patients’ needs. A treatment schedule will be set forth that the patient would be required to attend and participate in. We use evidence-based treatment modalities, complemented by cutting-edge counselling and therapies. As the patient is monitored closely throughout the treatment cycle, the primary therapist may make adjustments to their medications and/or therapies as needed.
Family Visits: Families have the opportunity and are encouraged to participate in the therapy process either by attending the scheduled family sessions or by participating in earned patient outings. By participating in family sessions, the loved ones and family members can also learn how to better understand the patient, their issues, challenges and disease. This kind of family involvement helps to enable the entire family to learn how to recover together.
Discharge: At the conclusion of the treatment, which typically lasts between 30 to 90 days, patients are discharged from the mental health rehab with a discharge plan consisting of continuing care strategies. At this time, the patient has learned about their mental illness and underlying circumstances, coping skills, and understands how to manage their condition.
During this stage, the primary therapist and case manager have worked closely with the patient to develop a suitable outpatient continuum of care. This may include IOP, Alumni Services and Sober Living arrangements in order to transition and continue the mental health recovery process.
Continuing Care: Medical Concierge understands that mental health recovery is a life-long process. Because of this, we encourage our patients to participate in alumni support services. We keep in touch with our patients via phone calls and text check ins and welcome back those who wish to be mentors to others in the recovery process. This inspires and encourages them to continue learning new coping strategies that makes their recovery lifelong.
For Healthcare Professionals/Referrals
Many Medical Concierge patients enter treatment at our mental illness treatment center by way of referrals. To ensure a smooth transition into treatment for such patients, we have streamlined the admissions process so that they receive treatment quickly, without having to navigate through paperwork or bureaucracy. We also make it easy for the referring health care professional to remain involved in the patients’ treatment, if they so desire.
Process
- Refer the patient to Medical Concierge by either calling our 24/7 helpline (877) 636-0042 or emailing us at info@mchealthrecovery.com.
- The treatment consultant would work with the referring doctor to build a patient profile and set an admit date.
- On the admit date, the paperwork, including consent forms and insurance policies, will be compiled and signed.
- A referral coordinator would inform the referring therapist within three days of the patient entering treatment. This would be an appropriate time to establish the nature and frequency of patient updates if the referring therapist desires.
By the conclusion of the treatment plan, which typically lasts between 30 to 90 days, the primary therapist and the case manager would have worked with the patient to develop a suitable and sustainable outpatient continuum of care, which could comprise either or all of IOP, Alumni Services or Sober Living arrangements. This would help the patient transition back to normal life and continue their mental health recovery.
